Hospital to Home

The Phoebe Joan Project Expands Support for Grieving Families.

30 September 2025.

This October, during Pregnancy and Infant Loss Awareness Month, we’re proud to share the profound impact of the Phoebe Joan Project, created in partnership with Red Nose Australia.

Since its inception, the project has directed almost $1 million into specialist bereavement care, ensuring that families in regional and remote communities receive the same level of compassionate, expert support as those in major cities.

Closing the Gap for Isolated Families.

For parents experiencing the devastation of stillbirth (including termination for medical reasons after 16 weeks), newborn loss or infant death, access to support can feel out of reach. The Phoebe Joan Project is changing this by funding online and phone-based services delivered by Bereavement Support Workers with lived experience of baby loss.

These services include:

  • One-on-one bereavement support in partnership with Red Nose’s Hospital to Home program, walking alongside families from the moment they leave hospital.

  • Ongoing care in the weeks and months that follow, including during subsequent pregnancies after loss.

  • Tailored support for families who often face greater isolation and fewer local services.

  • An additional service to support parents as they navigate the complexities and anxieties of subsequent pregnancies.

Standing Together This October

Every year, thousands of Australian families experience pregnancy and infant loss. The Phoebe Joan Project is helping ensure that no family has to walk through this heartbreak alone, no matter where they live.

Red Nose is calling on the community to join them this October in acknowledging the thousands of little lives lost each year to stillbirth, neonatal loss and TFMR, and to stand in solidarity with families who live with the grief of pregnancy and infant loss.

Red Nose Australia Hospital to
Home Regional Support

The Phoebe Joan Project (PJP) officially commenced in September 2021. The PJP was developed to address the gap in provision of short-term bereavement support to parents navigating the complex loss following the sudden death of a baby. Following on from Red Noses’ successful Hospital to Home program, the Phoebe Joan Foundation funded the project to specifically target parents in regional and remote Australia who may not have the same access to services offered in urban areas.

The Phoebe Joan Foundation is very proud of its ongoing support of the PJP which has proven results. In 2022, the Foundation increased its support to include additional resources for bereaved parents navigating the complexities and anxieties associated with subsequent pregnancies.
